Carlisle Area Little League: 2009 C.A.L.L.

Attached is a Medical Release form that must be completed and returned to your coach before Opening Day, April 18th.

Medical Release Form

All local Little Leagues are required to conduct background checks on any individual who provides regular service to the league and/or have repetitive access to, or contact with, players or teams. As a safety precaution to all children involved in the Carlisle Area Little League we must condut a background check on any volunteer. If you wish to participate as a volunteer please fill out the volunteer form and mail to the league with a copy of your photo identification.
