Coaches Registration

Coaches interested in playing in this benifit game please download, print and fill out this registration form and return it to the address at the bottom of the form.

Please include the 10 dollar registration fee with your registration form. Please make checks out to: Coaches vs. Cancer Soccer Classic (The NEW 10 dollar registration fee will go to cover both your playing shirts as well as other small game expenses)

You will also find that on the registration that you are asked to send a picture (Action or head shot). This can be done by e-mail by attaching the picture and sending it to HuskiesSoccer14@aol.com
