Each of these forms must be completed for tryouts. Membership and team information on the membership form will be completed by a club representative. Med History and Release does not need to be notarized.

09 USAV Membership Form

If you feel you're eligible for a reduced rate due to economic hardship you will receive a $30 discount on USAV registration which will reduce your base fees by the same. Filling out the reduced fee affidavit makes you eligible for a $100 grant from the Southern Region. Please notify Coach Akers if you intend to request this assistance.
