If You Haven't Filled Out a Player Record & Release ...
Players should download two attachments: Medical Liability & Photo Release and Player Record Form. If you're in a tournament, you may be asked to provide the player's birth certificate copy and drivers license copy (if applicable) and health insurance card (if available). These documents are sometimes requested by tournament officials.
Allow a few minutes to download forms. When completed, please send all paperwork to fax (949) 679-7324 or email to Claire@CAYardDawgsBaseball.com, or U.S. mail to 1 Lindberg, Irvine, CA, 92620.
The CA Yard Dawg uniform is one navy and one forest green jersey, and one cap. You are responsible for your navy socks, navy belt and white pair of pants. Navy undergarments are optional.
