2013 REGISTRATION
2013 MAIL IN REGISTRATIONS ARE STILL BEING ACCEPTED FOR FOOTBALL & CHEERLEADING
REGISTRATION FORMS CAN BE FOUND UNDER THE HANDOUT TAB AS WELL AS THE HOME PAGE.
PLEASE MAIL COMPLETED FORMS WITH REGISTRATION FEE TO THE PO BOX LISTED ON THE FORMS.
THE TOTAL REGISTRATION FEE FOR THE 2013 SEASON IS $160.00
**The 2013 Registration fee of $160.00 is for all levels of football and cheerleading (Peewee, Freshman, JV and Varsity)
THE REGISTRATION FEE BREAKDOWN:
$70.00 Registration, $25.00 Volunteer Fee, $25.00 PrePaid Raffle and $40.00 for Football Jersey,or $40 towards Cheerleading Uniform for Cheer registrants. Players keep their jersey.
The Volunteer fee is refunded once you fulfill your volunteer duties.
**NEW** OATKA REQUIRES A DOCTORS NOTE/PHYSICAL ALLOWING CHILD TO PARTICIPATE IN FOOTBALL/CHEERLEADING. THIS IS REQUIRED BEFORE THE FIRST PRACTICE**
**PLEASE HAVE REQUIRED DOCUMENTS, ALONG WITH PHYSCIAL, SMALL PHOTO AND COPY OF BIRTH CERTIFICATE TURNED IN BY FIRST PRACTICE**
