Brochures/Forms

The application forms for Club membership are available for downloading below as pdf files which can be printed out. The main form, 2008 Club Ice Flyer, includes the skating schedule from July 2008 through March 2009. Applications should also include the 2008 Parental Consent Form and the 2008 Liability Waiver Form.
