Please fill out a copy of the Player Registration Form and the Parents Code of Conduct for each player registered. For new players, you will need to provide a copy of the player's birth certificate at the time of registration.

BCYSA Coach's application form, and the Coach's Code of Conduct are both written in PDF format and require the Adobe Acrobat Reader in order to view it.

Note:The new coach application incorporates the SAY Volunteer Application Form and the new Volunteer Authorization/Consent form for background checks..

Two copies of each form, filled out and signed, are required at on-site registration.

Sponsors are needed to help offset the cost of uniforms, awards and plaques. We are a non-profit organization so donations are tax-deductable. As a sponsor you can have your company name screened on the player's jerseys along with their team name.
