We hold our parents to a high standard. That is why we require them to sign this form when registering their player.

This document describes exactly where the Little League patch should be put on the uniform.

Patch

This form or a similar one from their physician has to be filled out by a physician certifying that the player is healthy enough to play softball or baseball prior to the first game of the season. Without this form the player cannot play. The only exception is if the parent gives the manager an appointment slip for a physical within 30 days of the first game of the season.

Little League Insurance

This document details how a player needs to verify that they live within the boundaries of Antioch Little League. If they live outside the boundaries then form Reg II(d) must be filled out and given to the president of the league.

First aid for minor injuries

This document describes all of the practice fields that Antioch Little League uses. It has an aerial photo of them and some other photos. It is a good guide to use to determine which field you wish to practice on or how to assign teams to a field. Caution it is big file (greater than 2 MB).

Use this form if you find any safety problem or anything that needs to be repaired at AYSC. Turn it in at the AYSC snack bar or in the box at the mobile home. This form should not be used to report any problems that is under the control of the leagues using AYSC>
