Registration Forms

Team Registration: Includes Team name, colors, division, captain/rep information, and team roster. Each team must fill this out at the beginning of registration. Last update: 09/09/09

Player Registration: Once a player has a team to play with, the Team Rep must collect the following and ensure it is complete before forwarding to the AWSL Registrar:
a) payment
b) this application
c) copy of driver's license
d) expired player pass or digital picture
