How to file a Soccer Accident Insurance (SAI) Claim
The necessary forms for filing a Soccer Accident Insurance claim and necessary instructions can be found on the
Click on "AYSO Resources" on the top bar. Then select "Forms & Documents" from the left margin and click on "Insurance forms" in the pull down list. Select "Soccer Accident Insurance Claim form w/Instructions" to download the necessary forms and instructions.
