All 16U & 18U teams must submit the "Player Profile" form for their team by May 1, 2010 (you may submit a revised profile up to 2 weeks before the start of the tournament). Please return the completed for by email as an attachment.

The "Worksheet" is for the individual players to collect information. Please do not submit these sheets.
