Spring 2009 NorCal Premier League

     For teams playing NorCal Premier Spring 2009 season, please make sure your US Club Soccer rosters
     are up-to-date.  Any player changes, Coach, Assistant Coach, and Managers changes must be updated.
     Managers, please check your current roster and passcard rosters at www.usclubsoccer.com.

     US CLUB SOCCER Forms 

     #R002-Youth: Youth Player Registration & Medical Treatment Authorization Form - This is the form which
     is required of all players who are registering through their club with US Club Soccer. As an alternative to
     receiving this form from your club, players or parents may download the form from this site, complete it,
     and submit it to your club.

     Staff Background Check Application - Have each coach and administrator who work with players
     complete this online staff background check. This replaces the paper risk management form and is the
     only form necessary to submit for a staff member

2009 CYSA Membership Form 1601

CYSA Membership form to be completed for every player. Two copies need to be filled out, signed and turned in for every player. Fill-In & Print form and obtain the required wet-ink signature: This form MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over.   Players who are age 18 or over MUST sign their own form. If players turn 18 during the seasonal year, they must sign their current 1601 form. The wet ink signatures should be in a color.
