Please print out and bring 2 copies of this form with you to the 5-Star Hall. Form includes a medical waiver and acknoledgement of the new laws regarding concussion in youth sports. Please read the Concussion in Youth Sports handout prior to signing.

2009-2010 Registration

Concussion in Youth Sports- Every parent/gaurdian MUST read

5 Star has adopted a new policy for the management of concussion and head injury in youth sports; “The Lystedt Law” Requires coaches and parents to sign a waver indication they have read and agree with the mandatory policies for the management of concussions and head injuries as prescribed by HB 1824, section 2.
